If you are an organization already license to operate and is interested to participate in the implementation of humanitarian programs and projects all over United States of America and other countries thereafter, please fill out the form below and check the services your organization currently provide (check all that applies).

We will contact you when we conduct future seminar to familiarize you with the vision of H. MARTIN FOUNDATION so your organization will have a chance to sign a Preliminary Humanitarian Collaborative Agreement (HCA) with HMF which is similar to a Memorandum of Agreement (MOA) or Memorandum of Understanding ( MOU).

This Preliminary Humanitarian Collaborative Agreement (HCA) can be rescinded by either party at any time without incurring any obligation.
